What is POTS?
POTS — postural orthostatic tachycardia syndrome — is a form of dysautonomia in which the heart rate rises abnormally on standing without a corresponding drop in blood pressure. The standard adult criterion is a sustained increase of at least 30 beats per minute within ten minutes of standing, with symptoms lasting three months or more. Common symptoms include lightheadedness, palpitations, fatigue, brain fog, and exercise intolerance.
What’s the difference between POTS and dysautonomia?
Dysautonomia is the umbrella term for any dysfunction of the autonomic nervous system — the system that runs heart rate, blood pressure, digestion, and temperature without your input. POTS is one specific form of dysautonomia, defined by the heart-rate response to standing. Someone can have dysautonomia symptoms without meeting the POTS criteria, which is part of why evaluation matters.
How is POTS diagnosed?
POTS is diagnosed by measuring heart rate and blood pressure lying down and then standing, either with a stand test or a tilt-table test, and by ruling out other causes of the same symptoms — dehydration, anemia, thyroid disease, and medication effects among them. Is POTS related to Ehlers-Danlos syndrome and MCAS?
Frequently, yes. POTS, hypermobile Ehlers-Danlos syndrome, and mast cell activation syndrome overlap often enough that they’re commonly evaluated together, and many patients carry all three. Hypermobile connective tissue affects how blood vessels hold their tone, which is one proposed link. Bendavid Clinic evaluates for the overlapping conditions rather than treating the heart rate in isolation.
Can long COVID cause POTS?
Yes. POTS is one of the most commonly identified findings in people with long COVID, and post-viral dysautonomia has been documented after other infections as well. If your lightheadedness, racing heart, or exercise intolerance started after an illness you never fully recovered from, that history is directly relevant to the evaluation and worth bringing to your first visit.
How is POTS treated?
POTS is managed rather than cured, and treatment is built around the individual. Care commonly starts with non-drug foundations — increased fluid and sodium intake, compression garments, and a gradual recumbent-to-upright exercise progression — with medication considered when those aren’t enough. Which combination fits you depends on your symptoms, your other diagnoses, and what you’ve already tried.
Do I need a diagnosis or a referral before being seen?
No. You don’t need a referral or an existing POTS diagnosis to be seen at Bendavid Clinic — we see both patients who arrive already diagnosed and patients still looking for an explanation.
